Incident Report Policy

Policy

Cuan Bhríde Childcare Centre will record all incidents, accidents, and unusual behaviour that occur to children, staff, and visitors to the centre.

Procedure:

  1. An incident report will be used to record the incident/accident.

  2. The staff member involved in incident/accident will write the report. If no staff were involved, the staff member in charge of the room will write and sign the report.

  3. The report will be written objectively from the third person perspective.

  4. Following the Confidentiality Policy, no other child’s name may be named in the report except the child whom the report is about.

  5. At least one parent must be informed of the incident, either via telephone or in person on collection.

  6. A parent must sign the incident report. A copy of the report will be offered to the parent.

  7. The report, once signed, will go into the child’s file, which is locked at all times. Parents are permitted to see their child’s file upon request.

  8. If the report is about an adult, the report will be filed in a locked cabinet, following the Confidentiality Policy.
